Please remember to enter the name of your employer in the Company Name field. If you qualify through receipt of a means tested benefit, then please type ‘MTB’ here.
Once your membership has been processed, you will receive an SMS and/or email confirming when your card is ready for collection. If you don’t receive this within 24 hours, then please reply to this email.
